In these countries \,\nleishmaniasis remains embedded in the endemic villages where people\ne ither die or suffer for many months before seeking treatment. For\n2 years \, I have worked with the World Health Organization and\nleading experts a nd policy makers from India\, Nepal and Bangladesh\nto establish a unified strategy to eliminate leishmaniasis from\nthese countries. The eliminatio n target is a 95% reduction from the\ncurrent level of about 20 cases in 1 0\,000 in the highly endemic\nareas. Unique epidemiological features of th is disease in the\nIndian sub-continent make elimination possible\, includ ing\; (1)\nhumans are the only reservoir\, (2) only one Sandfly vector spe cies\,\n(3) the Sandfly is susceptible to insecticides and (4) the\ngeogra phic distribution is limited and highly clustered. The\nelimination strate gy must be simple\, using inexpensive and\neffective interventions that wo rk on a larger scale. The strategy\nwill combine the training of village A SHAs (village health\npersonnel) in active case finding in the endemic vil lages and\nholding fever camps. Suspect cases will be transported by ricks haw\nto primary health centers located within several kilometers of the\ne ndemic villages and treated with a 'single dose' of intravenous\nAmBisome that has been shown to be safe and provide a 95% cure\nrate. Therefore\, p atients will be diagnosed and treated on the same\nday near their village. This approach directly attacks the disease\nthrough actively reducing the human reservoir and reducing\ntransmission.
